What is Post-Acute Care?

Following a hospitalization or medical event for injury or illness, many patients require continued medical care, either at home or in a specialized facility. Post-Acute Care is a range of medical services that support the patientā€™s continued recovery from a recent injury or illness to ensure all issues are addressed and treated.

If you no longer need inpatient hospital care we may discharge you to a Post-Acute Care setting to assure you receive appropriate transitional care for continued recovery. To provide continuity of care, we have gathered the below information to help you choose a facility that works with a Florida Medical Clinic hospitalist. The guide includes helpful information about Home Health and Skilled Facilities that we believe provide excellent care.

What is Post-Acute Care?

Post-Acute Care is a range of medical services that support the patientā€™s continued recovery from a recent injury or illness to ensure all issues are addressed and treated. If you no longer need inpatient hospital care, we may discharge you to a Post-Acute Care setting to assure you receive appropriate transitional care for continued recovery.

What are the Benefits of Post-Acute Care?

Patients who are discharged to a Post-Acute Care Facility are less likely to be readmitted to the hospital. They are closely monitored for complications or condition changes. This improves the likelihood of catching new or recurring problems and treating them appropriately.

Will my Hospitalist see me in Post-Acute Care?

Your doctor is part of the Florida Medical Clinic transitional care team of Hospital Medicine doctors providing complete coordination across all venues and levels of care. This ensures continuity of care at all times including urgent visits if needed. You will be seen by one of our Florida Medical Clinic Hospitalist or Nurse Practitioner within 24-72 hours of admission based on the facility that you have chosen.
Choosing an in network post-acute care facility ensures that a Florida Medical Clinic provider will make your transition from hospital to post-acute care a seamless process. Additionally, continuous communication with your Florida Medical Clinic Community Providers is uninterrupted.

How much therapy will I receive each day?

Patients in a Skilled Nursing Facility will need to be able to tolerate therapy sessions that range from 30-120 minutes a day. The facilities offer multiple therapies that include: Physical, Occupational and Speech therapy. Therapy is performed seven days a week.

How long will I need to stay in Post-Acute Care?

Your length of stay varies depending on where you are at in your rehabilitation journey. Your Florida Medical Clinic provider will coordinate and communicate with the facility to determine the length of stay based on getting you back to your next safest level of function. The objective is to safely reach your clinical goals and get you back to healing at home within 10-15 days.

Can family members visit me during my stay?

Family members are welcome during visiting hours and are an important part of your rehabilitation. Each facility will have additional details regarding visiting hours. Visitors must be fully vaccinated, and face masks may be required. Most facilities allow no more than two visitors per resident.

How do I keep my family informed?

We believe that effective communication with your family is essential to patient care. With an entire care team composed of the Florida Medical Clinic Team as well as the facility team, we do our best to keep your family as informed as possible.

Does the facility I am considering accept my insurance?

The Admissions Team at each post-acute care facility will assist you in assuring you choose a facility that meets your needs and accepts your insurance.

Does the facility I am considering offer tours for my family?

When interviewing post-acute care facilities, you will meet with a friendly patient coordinator. Your coordinator will answer your questions and provide a facility tour for you and your family.

What should my family bring for me upon my arrival to Post Acute Care?

CLOTHING: should include athletic or non-skid shoes and comfortable clothes like t-shirts, sweatpants, shorts, underwear, socks, pajamas, robes, slippers with traction, etc.

TOILETRIES: specific personal hygiene products, hairbrush, shampoo and other hair products, makeup, denture adhesive, and cleaner if needed. Facility provides the basics: soap, toothbrush, comb, and toothpaste.

MEDICAL: name and contact of Primary Care Provider, current medication list, prescription plan ID cards, living will or advance directives.

What is the difference between inpatient and outpatient post-acute care?

Inpatient care provides you with round-the-clock care and treatment by medical professionals over a number of days. The focus will be on your recovery. Outpatient care does not include overnight stays.
