Dr. Cozzolino was born and raised in New York. He completed his medical school training at Albany Medical College in Albany, New York and his residency at Cone Memorial Hospital in Greensboro, North Carolina. From there, he joined the Family Health Clinic at Hurlburt Air Force Base in Florida, further cultavating his medical and leadership skills.
During his military commitment, Dr. Cozzolino completed his MBA degree to further his business, management, and analytical skills. However, his primary goal was to develop and maintain a successful patient-centered medical practice, which he started for Florida Medical Clinic in Wesley Chapel in 2002. In his spare time, Dr. Cozzolino enjoys spending time with his blended family and loving wife, Kim.

Hospital Follow-up Within 14 Days of Discharge

A hemoglobin A1c of >9.0% represents poor blood sugar control; therefore, the lower the value, the better. Patients who have a low hemoglobin A1c are less likely to suffer the side effects of diabetes

Depression Screening and Follow-up
