Hospital Follow-up within 14 days of discharge
The percentage of patients who are seen within 14 days after being discharged from the hospital. Studies show follow-up with a primary care doctor within 30 days of hospital discharges reduces hospital re-admission (returning back to the hosipital).
Breast Cancer Screening
Percentage of female patients 50 to 74 years old who had a mammogram. Early detection is important to prevent the spread of breast cancer.
Diabetic Poor Control Hemoglobin A1c > 9.0
Percentage of patients 18-75 years of age with diabetes who had hemoglobin A1c > 9.0% during the measurement period. A hemoglobin A1c of >9.0% represents poor blood sugar control; therefore, the lower the value, the better. Patients who have a low hemogloblin A1c are less likely to suffer the side effects of diabetes.
Percentage of patients 65 years of age and older who have ever received a pneumococcal vaccine. Vaccination is a proven method to prevent the spread of pneumonia.
Colorectal Cancer Screening
The percentage of patients 50 to 74 who received colorectal cancer screening. Early detection is key to preventing the spead of colon cancer.
Advanced Care Planning
Percentage of patients aged 65 years and older who have an advance care plan or surrogate decision maker documented in the medical record or documentation in the medical record that an advance care plan was discussed but the patient did not wish or was not able to name a surrogate decision maker or provide an advance care plan.
