Dr. Sirchia has provided medical care to the Brandon, Riverview and surrounding communities since 1995. He is one of the founding partners and an instrumental figure in the growth of the Family Medicine of Riverview. The practice merged in 2013 with Florida Medical Clinic to enhance patient care, bringing cutting edge medicine and more medical options for area families.
With over twenty-three years of experience in medicine his unique style of practice reflects the basic foundation of health and prevention. All while incorporating current evidence based medicine and therapeutics to all patients. Dr. Sirchia holds advanced certifications in Trauma Life Support and Cardiac Life Support. He is involved with local groups of the American Medical Association. He enjoys sports and is active in racquetball and soccer. Dr. Sirchia lives in the area with his wife and two daughters.

Hospital Follow-up Within 14 Days of Discharge
Percentage of patients who were seen by their primary care provider (PCP) within 14 days of a hospital discharge. Following up with a PCP has been linked to reducing re-admission to the hospital.
Breast Cancer Screening
Percentage of female patients 50 to 74 years old who had a mammogram. Early detection is important to prevent the spread of breast cancer.
Colorectal Cancer Screening
Percentage of patients 50 to 75 years old who had an appropriate form of colorectal cancer screening. Early detection key to surviving the disease.
Diabetic Poor Control Hemoglobin A1c > 9.0
Percentage of patients 18-75 years of age with diabetes who had hemoglobin A1c > 9.0% during the measurement period. A hemoglobin A1c of >9.0% represents poor blood sugar control; therefore, the lower the value, the better. Patients who have a low hemoglobin A1c are less likely to suffer the side effects of diabetes
Influenza Vaccine
Percentage of patients 6 months or older who had an influenza immunization. Vaccination is a proven method to prevent the spread of the flu
Pneumonia Vaccine
Percentage of patients 65 years of age and older who have ever received a pneumococcal vaccine. Vaccination is a proven method to prevent the spread of pneumonia
Depression Screening and Follow-up
Percentage of patients 13 and old who received a depression screening and a follow-up plan if the patient tested positive for depression. Depression is often missed or overlooked, annual screenings enables health care providers to detect and treat depression.
Advanced Care Planning
Percentage of patients aged 65 years and older who have an advance care plan or surrogate decision maker documented in the medical record or documentation in the medical record that an advance care plan was discussed but the patient did not wish or was not able to name a surrogate decision maker or provide an advance care plan.
Urgent Care Utilization During Office Hours (M-F 8AM-5PM)
Percentage of patients who had an Urgent Care Visit during their PCP’s normal office hours (Monday – Friday; 8:00AM – 5:00PM). Patients who seek care from other providers, such as urgent care, during their PCP’s normal office hours may indicate the PCP does have enough appointment access, or the patient’s perception is their PCP does not have adequate availability. The lower the number, the better.
