Dr. Forman grew up in Ft. Lauderdale, FL and moved to the Tampa Bay area in 1998. In his spare time, he enjoys playing golf and tennis, along with learning Tae Kwon Do with his daughter. Dr. Forman is the Medical Director of both Zephyrhills and Wesley Chapel Urgent Care Facilities.

I have had to take my elderly father to the urgent clinic many times in the last few years. Here we have had the pleasure of being treated by Dr. Forman. Aside from being a brilliant doctor, he has empathy and takes his time to listen to the patients rather than rushing through their problems like so many other physicians do these days. My father said that just seeing Dr. Forman makes him feel better.

Although Dr. Forman is considered "Urgent Care", I think I see him more than I see my primary! Dr. Forman is fast, thorough and caring! He gets you in and out without feeling as if there was a lack of care, will schedule you for a follow up to see him if he thinks you need it and always tries to get to know you as a person and then remembers when you come back at a different time! Not to mention, he used to be an ER doctor AND his hours are extremely convenient! I have been seeing Dr. Forman for over 10 years and will continue to, because I trust him with my healthcare!
