Our medical staff helps patients prepare and file the appropriate insurance claim forms. Patients with billing questions should call our Patient Representatives in Zephyrhills at 813.782.7940 or in Tampa at 813.971.4544 or toll free at 1.800.894.7090.  Florida Medical Clinic accepts Medicare assignment and will file patients Medicare claims and will also file supplemental claim forms.
